Your Opportunity: Social Worker (MSW), PRN

We are seeking a compassionate professional with a strong commitment to collaborate closely with nursing, therapy, physicians and other allowed practitioners, clients, families, caregivers, and community providers to support safe, coordinated, patient-centered care..

In this role, you will plan an important role in supporting the psychosocial, emotional, financial and community resource needs of our clients .

The Health Information Coordinator plays an important role in supporting Solara’s mission to deliver exceptional, person-centered care while upholding the highest standards of clinical accuracy, confidentiality, and organizational integrity.

Key Responsibilities:
  • Conduct comprehensive psychosocial assessments to identify social, emotional, environmental, financial, caregiver, and community factors that may affect the client's health, safety, treatment, recovery, or ability to remain safe.
  • Participate in the development, implementation, evaluation, and revision of the individualized plan of care in collaboration with the client, family or caregiver, physician or allowed practitioner, and the interdisciplinary care team.
  • Provide short-term supportive counseling and psychosocial interventions related to the client's illness, treatment, adjustment, or home care needs
  • Provide professional, courteous support to clients, families, physicians, and team members.
  • Assist clients and families with long-term care planning and connect them with appropriate community or governmental resources when needs extend beyond the scope of Medicare home health services.
  • Maintain accurate, timely, compliant, and complete documentation of assessments, interventions, client and caregiver education, resource referrals, communications, progress towards goals, and discharge planning.
 
Qualifications:
  • Meets applicable federal and Florida requirements to provide medical social work services within a Medicare-certified home health agency.
  • Previous experience in healthcare, home health, hospice, geriatrics, rehabilitation, hospital social work, case management, or related healthcare environment.
  • Demonstrated ability to assess complex psychosocial situations and develop appropriate interventions within the scope of home health services.
 
Minimum Education Required:
Master's degree in Social Work (MSW) from an accredited educational institution.
 
Experience:
Previous experience in healthcare, home health, hospice, geriatrics, rehabilitation, hospital social work, case management, or related healthcare environment.
