Your Opportunity
We are seeking an PRN Physical Therapist Assistant who is responsible to assist in implementing treatment plans, documenting client progress, ensuring compliance with all professional standards, regulatory requirements, and Solara Care Solutions policies. This role is essential to supporting independence, dignity, and quality of life for those we serve.

Key Responsibilities:
- Assis with client screenings, including new admissions within 24 hours and long term residents experiencing significant change or referred by nursing
- Implement treatment programs for clients as directed by the Physical Therapist and supported by doctor's orders.
- Complete all required documentation accurately and timely. This may include daily treatment notes, weekly progress notes, monthly recertifications, and discharge summaries, all co-signed by the supervising Physical Therapist
- Schedule clients appropriately on the assigned caseload.
Qualifications
Minimum Education Required:
Completion of an accredited Physical Therapist Assistant program
