We are seeking a Physical Therapist [PRN] whose responsible for providing direct patient care through skilled evaluation, treatment, and documentation in accordance with all professional, state, and federal standards.

Key Responsibilities: 

  • Responsible for the evaluation and treatment of residents who demonstrate disorders of musculoskeletal deficiencies, neurological conditions, balance disorders, and transfer/gait deficits. Evaluation and treatment will only be conducted after a written order is received from the physician.

  • Responsible for the screening of residents of the community as follows: New Community residents: Within 24 hours of admission Long term residents: Upon receipt of a "significant change" form and/or upon referral from nursing.
