We are seeking a Physical Therapist[PRN] whose primary responsible for  their direct clinical skills, documentation skills, and the compliance to all professional standards.

Key Responsibilities: 

Responsible for the evaluation and treatment of residents who demonstrate disorders of musculoskeletal deficiencies, neurological conditions, balance disorders, and transfer/gait deficits. Evaluation and treatment will only be conducted after a written order is received from the physician.

  • Responsible for the screening of residents of the community as follows: New Community residents: Within 24 hours of admission Long term residents: Upon receipt of a "significant change" form and/or upon referral from nursing.
  • Responsible for attending all required community meetings and in-services, including Medicare meetings, family meetings, care plans

QUALIFICATIONS: 

Minimum Education Required: 

Masters of Science Degree or Bachelor of Science Degree in Physical Therapy. State Licensure.

 

Experience: 

As applicable to assignment
