Your Opportunity
Fleet Landing is seeking a highly motivated, confident, self-started to be our Intake Coordinator who wills serve as the primary point of contact for Residents, families, hospitals, and referral sources seeking short term rehabilitation services. As the Intake Coordinator you will manage the admissions process from inquiry to arrival, ensuring a smooth transition and exceptional customer service as well as assisting with tasks within the unit. You will work closely with the clinical, financial, and administrative teams to asses eligibility, coordinate care, and maintain census goals.

Responsibilities:
The Intake Coordinator serves as the primary point of contact for families, hospitals, and referral sources who are seeking short-term rehabilitation services.
Key Duties:
- Respond promptly and professionally to all admission inquiries from hospitals, physicians, families, and referral partners.
- Maintains accurate records in the admissions database and prepares regular census. admissions reports.
- Provides tours of the community and detailed explanations of services, amenities, and admission requirements.
- Review admissions packets with Residents, families, and/or responsible parties to confirm understanding of rights, services, and financial obligations.
- Coordinates and schedules appointments as directed.
- Collaborate with the interdisciplinary team to review referrals for clinical and financial approval.
- Coordinates bed availability, room assignments, and arrival arrangements for incoming residents.
Qualifications:
Minimum Education Required:
High school diploma or equivalent is required. Associate's or Bachelor's degree in business, healthcare administration, or related field preferred.
Experience:
Prior experience in healthcare admissions, discharge planning, case management, or skilled nursing operations is strongly preferred
Employment Variables:
Must be able to work flexible schedule; nights, weekends, some holidays as designated.
