Your Opportunity
Fleet Landing is seeking a highly motivated, confident, self-started to be our Home Health Liaison. The Home Health Liaison will develop and maintain relationships with referral sources such as hospitals, physicians, skilled nursing facilities, assisted living facilities, and other healthcare providers to drive census growth. position are developing and maintaining referral sources, promoting and communicating the philosophy of Fleet Landing an referring appropriate clients to our health services programs.

Responsibilities:
The Home Health Liaison is responsible for planning and implementing marketing and sales activities for Fleet Landing Home Health. Major functions include developing and maintaining referral sources, service coordination, promoting and communicating the image and philosophy of Fleet Landing, sales support to programs, and referral of clients to appropriate and desired services.
Key Duties:
- Develop new referral sources as needed to achieve Episode count and Private Duty referrals.
- Develop and maintain contact with key, targeted referral sources.
- Track and report marketing activities, referral data, and conversion rates.
- Achievement of individual sales/volume goals as developed and prioritized by Health Services leadership.
- Participate in activities and events that promote the image of Fleet Landing.
- Develop the annual Marketing Plan with the Health Services leadership and participates in ongoing planning and marketing evaluation activities, including: ongoing competitive analysis, marketing needs assessments, and tracking of referrals.
Qualifications:
Knowledge of Medicare home health guidelines and regulations is strongly preferred. Proficient in CRM tools and Microsoft Office Suite. Must possess excellent communication skills and the ability to work independently.
Minimum Education Required:
Bachelor's Degree in Marketing, Business, Communications, Healthcare Administration or a related field preferred
Experience:
Two (2) years of marketing or business development experience in a home health environment.
Employment Variables:
Must be able to work flexible schedule; nights, weekends, some holidays as designated by the Senior Director of Health Service.
