We are seeking a Speech Language Pathologist [PRN] whose responsible for providing high-quality evaluation, treatment and documentation services to residents with communication and swallowing disorders, in accordance with professional standards and regulatory requirements.

Key Responsibilities: 

Evaluates and treats residents with speech, language, cognitive-communication, voice, fluency, and swallowing disorders
Conducts resident screenings per admission and change-of-condition guidelines
Completes all required clinical documentation timely and accurately
Attends required meetings including Medicare, family, and care plan meetings
Develops individualized treatment programs
 

QUALIFICATIONS: 

Minimum Education Required: 

Master's degree in Speech Language Pathology. State Licensure and ASHA certification or eligibility

 

Experience: 

As applicable to assignment
