Key Responsibilities:
  1. Supports the mission, vision, and core values of Fleet Landing.
  2. Assures that Residents’ rights to fair and equitable treatment, self determination, individuality, privacy, property and civil rights, including the right to register complaints, are followed.
  3. Follows all established safety procedures and precautions.  Reports all unsafe/ hazardous conditions, and/ or defective equipment to Manager.  Reports all incidents/accidents, no matter how minor, to Manager.
  4. Provide services in accordance with accepted standards of practice for occupational therapy.
  5. Document all therapy interventions
  6. Provide initial and ongoing assessment and evaluation of patient/family response to therapy
  7. Develop and revise the plan of care in consultation with the physician and other care-team members.
  8. Provide therapeutic treatments and evaluation of equipment needs as applicable.
  9. Advise and consult with the family and other agency personnel as applicable
  10. Complete documentation timely for all assessment and evaluation data, treatments, and patient’s response to therapy intervention.
  11. Evaluate outcomes of care and complete the OASIS assessment accurately.
  12. Provide discharge planning and evaluation.
  13. Supervise occupational therapy assistants at least every 30 days and home health aides every 14 days when appropriate.
  14. Participate in in-service programs.
  15. Performs other duties as assigned by Manager.
Demonstrated ability to read, write, and speak English. Must possess good verbal and written communication skills. Must be able to meet deadlines, work under limited supervision. Must be able to climb at least 2 flights of stairs and be able to stand/or walk for a period of at least 4 hours. Valid state driver’s license. Motor Vehicle Driving Record satisfactory to the Agency and its insurance providers. Proof of motor vehicle insurance.

Minimum Education Required:
Bachelor of Science Degree in Occupational Therapy. State Licensure. Current CPR
