Fleet Landing currently has over 140 healthcare beds including a post-acute unit, long-term care, assisted living, and memory care. Our latest expansion, Beacon Pointe at Fleet Landing, will bring 128 new Independent Living Residences including 30 new skilled nursing beds, and 38 new assisted living to our thriving community!
 
Fleet Landing is seeking a Social Services Coordinator who is responsible for organizing Social Services to meet the social and emotional needs of Health Center Residents. Acts as a referral source to Residents and family members. Answers Resident and family member’s questions regarding the Health Center and its services. Assists Residents in functions outside the facility as directed. Works with Resident and family members to optimize the Health Center experience.

Key Responsibilities:
  • Responsible for all Resident admissions to the Health Center.
  • The establishment of a working relationship with the local hospital discharge planners to facilitate resident admissions from the hospital. 
  • Greets new Residents and identifies their immediate needs; e.g. clothing, telephone, newspaper and mail transfer.
  • Orients new Residents and family members to the Health Center.
  • Completes a Social History, Initial Discharge Plan and Advance Directive Check List on admission for assigned Residents.
  • Assist in the care plan process by completing MDS, RAPs and care plan for assigned Residents.
  • Attends care plan meetings.
  • Participates in the Continuous Quality Improvement meetings monthly/quarterly.
  • Responsible for fulfilling the role of “Abuse Investigator” for the Health Center residents and staff members. 
  • Maintains regular progress notes indicating response to the treatment plan and adjustment to the Health Center for assigned Residents.
  • Assists in the development of discharge plans for Residents and family members as directed by the Senior Director of Health Care Operations.
 
QUALIFICATIONS:
 
Minimum Education Required:
Bachelor’s Degree in Social Work or related field.
 
Experience:
Prior experience in a skilled nursing facility or in a geriatric setting.
