Our Opportunity
We are seeking a Occupational Therapist Assistant who provides services in client’s home. The Occupational Therapist Assistant  is responsible for the safe and effective delivery of those services or tasks delegated by and under the direction of the occupational therapist. Communicates client changes to occupational therapist.

Key Responsibilities:
  • Participation in the evaluation or assessment of a patient by gathering data, administering tests, and reporting observations and client capacities to the occupational therapist.
  • Participation in intervention planning, implementation, and review.
  • Implementation of interventions as determined and assigned by the occupational therapist.
  • Documentation of patient responses to interventions and consultation with the occupational therapist about patient functionality.
  • Assistance in the formulation of the discharge summary and follow-up plans.
  • Implementation of outcome measurements and provision of needed patient discharge resources.
Demonstrated ability to read, write, and speak English. Must possess good verbal and written communication skills. Must be able to meet deadlines, work under limited supervision. Must be able to climb at least 2 flights of stairs and be able to stand/or walk for a period of at least 4 hours. Valid state driver’s license. Motor Vehicle Driving Record satisfactory to the Agency and its insurance providers. Proof of motor vehicle insurance.
Minimum Education Required:
One year of experience, preferably in hospice, home health, or a skilled nursing facility. Completion of 2 years Certified Occupational Therapy Assistant program.  State Licensure.
