Responsibilities:
The Home Health Liaison is responsible for planning and implementing marketing and sales activities for Fleet Landing home health programs in coordination with the Senior Director of Health Services and the Home Health Leadership. Major functions include developing and maintaining referral sources, tracking referral activity, promoting and communicating the image and philosophy of Fleet Landing and referral of clients to appropriate and desired services. 
 
 
Key Duties:
  • Supports the mission, vision, and core values of Fleet Landing. 
  • In coordination with Senior Director of Health Services and Home Health Administrator, develops strategic plan to develop referrals for home health. 
  • Per strategic plan, develops and maintains contact with key, targeted referral sources toward the generation of new referrals and maintenance of obtained referral sources. 
  • Provides referral sources with current information on services. 
  • Responsible for achievement of home health episodes each month. Maintains record of activities. 
  • Coordinates and facilitates client referral and intake up to and including: preadmission review of clients, obtaining written orders, face to face information and packets for admission to home health.
  • Performs competitive analysis, marketing needs assessments and tracking of referrals per strategic plan.
  • Attends local training as needed on sales and marketing principles methods
  • Maintains current knowledge of funding and government regulations.
  • Performs other duties as assigned by Manager. 
 
Qualifications:
 
Minimum Education Required:
Bachelor's Degree or equivalent in related field preferred. 
 
Experience:
One (1) year experience in home health environment. Experience in marketing activities, preferably in the home health or hospice environment. Two (2) years of sales experience desired, preferably in home health or hospice environment. Valid state driver's license required. Motor Vehicle Driving Record satisfactory to the agency and its insurance providers. Proof of motor vehicle insurance required. 
 
Employment Variables:
Must be able to work flexible schedule; nights, weekends, some holidays as designated by the Senior Director of Health Services. 
 
Supervision
Reports directly to the Home Health Administrator.
